SPA SERVICES
Terms for membership:
· Services are to be received within each calendar month. Unused services may be rolled over for a maximum of 2 months.
· VIP Membership – Bank your bucks for up to 6 months.
· All memberships, unless otherwise stated are a 6 month term.
· Dues paid on the 1st of the month.
· Subsequent month dues will be charged to the credit card on file the first day of the month.
· Appointment cancellations require 48 hour notice via telephone (360-232-8430). If Service is not cancelled at least 48 hours prior, the client will be charged for the service.
· If membership is cancelled prior to the specified terms, a cancellation fee of $199 will be charged.
· If cancelling with fewer than specified months of membership, any promotional items received will be charged in full.
· Memberships will continue on a month to month term after the initial term.
· Membership changes or cancellations require 15-day written notice you may contact SW WA ESTHETICS Institute at swesthetics.institute@gmail.com or 360-232-8430 to obtain the cancellation form.
· $50 bank fee will be charged to the card on file if the card fails on the due date. A second bank draft will be attempted.
